You were disgusted, so at whose hands do you lay the blame? Are you expecting the prison guards and ancillary staff to risk attack by entering her cell in the normal manner, did you expect better of the adoptive parents in their child-rearing skills (I can hear the remarks of Pamela Boutflour wondering how it could all have happened echoing in my ears), are the rule makers who are not in situ more interested in covering their own backs than the fate of any one individual, or did we as a society all connive in her destruction?

Moving forward these were the recommendations following her death:

Smith's death be used as a case study to demonstrate how health care and the Correctional Service of Canada (CSC) failed her.
Within 72 hours of admission to a federal institution, all female inmates be assessed by a psychologist to determine whether self-injurious behaviour exists.
Female inmates receive support from female psychologists and support workers.
The CSC ensure nursing services are available on site for inmates at all times.
There be adequate staffing of qualified mental health staff at every women's institution.
The CSC expand the scope and terms of psychiatrists' contracts to enable them to perform duties in a meaningful way.
A federally operated treatment facility for high-needs, high-risk women be created.
Decisions about clinical management of inmates be made by doctors, not CSC staff.
Inmates must have access to an independent patient advocate system
Indefinite solitary confinement for prisoners be abolished.
Until indefinite solitary confinement is abolished with CSC, its use must be restricted to no more than 360 hours.
Meetings between prisoners and support staff should not happen through food slots (something that happened frequently with Smith.)
Prison staff be allowed to refuse orders without fear of reprisal.
Prison staff at all levels be personally responsible for everyone's right to life.
Female inmates be accommodated in regions closest to family and supports.

Sorry but it's not within the remit of any public sector worker whether in schools, prisons A&E on a Saturday night to be Aunt Sallies for this type of person. It should never have come to this stage that she was so damaged but given the situation that they found themselves in the guards at the chalk face had the right to take the measures they thought fit.

They are not riot Police but prison employees who have a right to shut their front door at the end of the day and switch off. They also have a right not to be put unnecessarily in harm's way.

I don't think she did ever attack anyone physically apart from throwing apples at a postman (this was on a home visit) and throwing toilet water in a youth worker's face, but she had hundreds of anti-social behaviours to her record, which is what landed her in the institution in the first place. The prison guards were entitled not to be assaulted every time they opened her cell and to take the necessary precautions to ensure this did not happen.

Well she was unpredictable. There were incidents of her putting glass fragments in her hair and smothering faeces on the wall of her cell. You have to think of the employees, who are just as human as anyone and entitled to reasonable working conditions.

However I do agree that the "pass the parcel" approach to her case was unacceptable and may have exacerbated her condition. http://www.cbc.ca/news/canada/toronto/isolation-mental-health-probed-in-ashley-smith-jail-death-1.1194367

I don't know how you expect the prison guards to handle someone who would have assault on her record even if it was throwing apples at a postman. They had called the Police several times before to deal with her and I suppose in the end they were issued with pepper spray. I don't like it just as much as you and I think she should have been transferred to a mental health facility and not an adult prison when she turned 18, but these were decisions taken over the heads of those at the chalk face.

But there were hundreds of anti-social incidents before she was taken into custody. I agree that the mental health provision was woefully inadequate and the judge at the inquest who had all the facts at his disposal ruled her death a homicide.

I'm sorry if it sounded as if I lacked sympathy for the girl. She was looking for the attention she felt she never received. All I would say is that the job of prison guard must be stressful enough without having to deal with all the problems this girl posed.
